Health Coach (Fully Remote)
About the position
The Health Coach at CVS Health plays a vital role in supporting Medicare members through Aetna's Healthy Aging Support Program. This fully remote position focuses on providing health coaching, education, and resources to help members manage their health, particularly those with chronic conditions. The Health Coach will facilitate group coaching sessions, assist members in engaging with their healthcare providers, and document interactions to ensure comprehensive support.
Responsibilities
? Educate members on topics such as Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams.

? Assist members in actively participating with their healthcare providers and Aetna Care Management teams regarding health decisions.

? Document all member interactions into the appropriate member records.
,
? Demonstrate empathy for situations faced by members and their caregivers.
,
? Troubleshoot unavailability of resources and secure support to close member gaps.
,
? Follow program workflows consistently and follow up with members to review personal action plans.
Requirements
? At least 2 years working as a Health Coach.

? Health Coach certification is required.
,
? Availability to support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM Pacific Time.
Nice-to-haves
? Community health or public health experience preferred.

? Managed Care/Medicare/Medicaid experience preferred.

? Bilingual preferred.

? Bachelor's degree preferred.
