What to Expect (Job Responsibilities):
• Process referral requests from care coaches to behavioral health providers and health plan resources
• Coordinate appointment scheduling for behavioral health provider visits, providing support to members
• Assist members in initiating their program journey by providing essential resources
• Monitor and respond to changes in member plan eligibility for enrollment updates
• Review and upload session notes and documents from provider offices to member charts

What is Required (Qualifications):
• High school diploma required; Associate's Degree preferred
• Minimum of 1 year of experience in healthcare; behavioral health experience is a plus
• Proficiency in Microsoft Office (Word, Excel, PowerPoint) and healthcare documentation systems
• Ability to communicate effectively and courteously while handling multiple tasks
• Reliable high-speed internet connection with specified download and upload speeds
