PRN RN Personal Care Screener (Remote with In-Home Visits)

We are seeking a compassionate and detail-oriented Registered Nurse (RN) Personal Care Screener to join our team. This position involves conducting in-home functional assessments to determine eligibility and need for Personal Care Worker (PCW) services. The RN Screener will conduct the Personal Care Screening Tool (PCST) assessment, by observing individuals performing Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), and submit documentation to support prior authorization requests. This is primarily a remote position with required in-home visits for client assessments. Qualifications License: • Current, Wisconsin Registered Nurse (RN) license. Experience: • Minimum of two (2) years of nursing experience. • At least one (1) year of experience in home health or in serving individuals with developmental disabilities, physical disabilities, or the elderly. Skills: • Proficiency in computer applications, including Microsoft Office, Adobe and electronic health record (EHR) systems. • Strong written and verbal communication skills. • Ability to conduct thorough in-person assessments while adapting to diverse populations and environments. Strong organizational skills with attention to accuracy and detail. Key Responsibilities • Conduct in-home evaluations to observe individuals completing ADLs and IADLs to determine their functional ability and level of independence. • Administer the Personal Care Screening Tool (PCST) in compliance with state and program guidelines. • Assess the individual’s need for a Personal Care Worker (PCW) and document clinical justification. • Submit accurate, timely documentation to support prior authorization of services. • Promote independence by encouraging the use of assistive devices and teaching alternative techniques where appropriate. • Ensure assessments exclude informal caregiver support (family/friends) when determining PCW needs. Maintain compliance with state, federal, and agency policies regarding assessments and documentation.
