The Assessment & Referral Counselor is responsible for:
? Conduct Mobile Assessments and review for clinical disposition
? Crisis Intervention
? Triage as needed
? Completes assessments and provides referrals if necessary
? Interface closely with Medical Staff, Treatment Team, and Managed Care Organizations

Qualifications

Requirements: ? Master's Degree in Social Work or related field; or be a ? Registered Nurse licensed in the State of Indiana ? Computer proficiency ? Good listening and communication skills ?
