The Licensed Practical Nurse is responsible for utilizing the nursing process to provide for optimum quality and continuity of nursing care to patients in the comfort of their residence.

Responsibilities
? Complete and scan required record keeping forms/charts upon the guest?s admission, transfer, and/or discharge.
? Chart nurses? notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest?s response to the care.
? Report all discrepancies noted concerning physician?s orders, diet change, charting error, etc., to the Director of Nursing.
? Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest?s medical record in accordance with established procedures.

Qualifications
? Current, unencumbered LPN/LVN license required or Compact State license- required.
? A minimum of 2 years LPN/LVN experience required.
? Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
