Description:

CVS Health is seeking a dedicated and experienced Remote RN Care Manager to join our dynamic team. As a Remote RN Care Manager, you will be responsible for managing and coordinating care for our patients, ensuring they receive the highest quality of care in a timely and efficient manner. This role allows you to work from the comfort of your home, providing you with a flexible working environment that promotes a healthy work-life balance.

The ideal candidate will be a Registered Nurse with excellent communication skills, a strong understanding of clinical practices, and a passion for helping others. You will work closely with patients, their families, and a team of healthcare professionals to develop and implement personalized care plans that meet each patient’s unique needs.

Requirements:

• Active Registered Nurse (RN) license in the state of practice
• Minimum of 2 years of clinical experience
• Previous experience in case management or care coordination
• Excellent communication and interpersonal skills
• Proficient in using electronic health records (EHR) systems
• Ability to work independently and manage time effectively
• Strong problem-solving skills and attention to detail

Responsibilities:

• Assess patients’ health conditions and develop individualized care plans
• Coordinate and facilitate care transitions among healthcare providers
• Monitor patients’ progress and adjust care plans as needed
• Educate patients and their families about their health conditions and treatment options
• Collaborate with healthcare team members to ensure comprehensive care delivery
• Maintain accurate and up-to-date patient records
• Provide emotional support and guidance to patients and their families

Educational Qualifications:

• Bachelor’s degree in Nursing (BSN) preferred
• Active RN license in the state of practice

Experience:

• Minimum of 2 years of relevant clinical experience
• Experience in case management or care coordination preferred
