Healthy First Steps is Florida?s Maternity Case Management program for Community & State/United Health Care, and we are looking for passionate LCSW?s who want to provide Care Coordination and act as a resource for information about and referral to other community-based services to our Medicaid members and their unborn babies to ensure they have a healthy and uneventful pregnancy and delivery!

This is a work from home position working with the Healthy First Steps Program. Employee must reside in the state of Florida. This position will be mostly telephonic, with some potential for field-based work, going into provider offices and working with high-risk OB population.

If you are located within the following counties: Miami-Dade, Volusia, Flagler, Marion, Alachua, Polk, Hillsborough, Highlands, or Lake, FL, you will have the flexibility to work remotely as you take on some tough challenges. Primary Responsibilities: ? Assesses patient and family psychosocial needs and develops plan of care in concert with patient, physicians, nurses and other members of the departmental team. ? Links patients/family to appropriate community resources including but not limited to, information/referral to sources of financial assistance, transportation, support groups, and other community services. ? Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical record. ? Serves as patient advocate and liaison with physicians, families, insurance company, community agencies and others as needed to ensure continuity of care. ? Provides discharge planning based on meeting identified goals of the treatment plan.

Required Qualifications: ? Licensed Master?s Degree-level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling - LCSW. ? Active and unrestricted license in FL. ? 1+ years of experience working with maternal/child nursing. ? Intermediate level of experience working or volunteering within the local community, with demonstrated knowledge of culture and values within the community and familiarity with the resources. ? Intermediate level of computer proficiency including the use of Microsoft Outlook, Teams, and multiple web applications. ? Florida resident and reside within a commutable distance to Gainesville, FL or Tampa, FL. ? Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a United Health Group approved high-speed internet connection or leverage an existing high-speed internet service.

Preferred Qualifications: ? Knowledge of community resources and organizations to meet member psychosocial needs. ? Knowledge of Florida Community Maternal Child Resources. ? Bilingual skills - fluency in Spanish/English or Creole/English. ? All Telecommuters will be required to adhere to United Health Group?s Telecommuter Policy.

The salary range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. United Health Group complies with all minimum wage laws as applicable.
