Job Description

We are seeking a dedicated and compassionate Virtual RN Case Manager to join our team at CVS Health. Requirements:

• Active and unrestricted RN license in the state of residence
• Minimum of 3 years of clinical nursing experience
• Experience in case management or care coordination preferred
• Strong communication and interpersonal skills
• Proficiency in using electronic health records and other healthcare technology
• Ability to work independently and manage a caseload effectively
• Reliable internet connection and a dedicated home workspace

Responsibilities:

• Conduct comprehensive assessments of patients’ health status and needs
• Develop individualized care plans in collaboration with patients, families, and healthcare providers
• Coordinate and monitor the implementation of care plans to ensure continuity of care
• Provide education and support to patients and families regarding healthcare options and resources
• Utilize telehealth technologies to conduct virtual visits and follow-ups
• Document all patient interactions and care planning activities accurately and timely
• Participate in team meetings and contribute to quality improvement initiatives

Educational Qualifications:

• Associate’s or Bachelor’s degree in Nursing (BSN preferred)
• Current and active RN license

Experience:

• Minimum of 3 years of clinical nursing experience
• Experience in case management, care coordination, or related field preferred
